Fit for Life Wellness & Rehabilitation is a clean, modern and large facility. At Fit for Life Wellness & Rehabilitation Centre we believe care should encompass 3 important pillars.
1. Interprofessional Collaboration?
Interprofessional collaboration is the process of developing and maintaining effective interprofessional working relationships with practitioners, patients/clients/families and communities to enable optimal health outcomes.
Simply put, interprofessional collaboration is when two or more practitioners work together on a patient case. The purpose and benefit of collaboration is to ensure that excellent service is provided to the patient. By working together across professions, we are pooling knowledge and making strides to ensure that patients receive appropriate and necessary treatment(s). At Fit for Life Wellness and Rehabilitation Centre, the wellness and rehabilitation providers work along side the family medical doctors and pharmacy team. We are proud of this “one stop shop” collaborative relationship.

2. Patient Focused Health Care?
The Ontario Medical Association definition of patient-focused care is as follows: “A patient-centred care system is one where patients can move freely along a care pathway without regard to which physician, other health-care provider, institution or community resource they need at that moment in time. The system is one that considers the individual needs of patients and treats them with respect and dignity.” At Fit for Life Wellness and Rehabilitation Centre, we believe that the patient needs to have access to information about a variety of health care options. We believe in open communication with the patient. Furthermore, we include the patient as apart of our team and decision making process.

3. Evidence Directed Care?
At Fit for Life Wellness and Rehabilitation Centre, we believe the treatment and care should be directed by evidence and the latest treatment guidelines. We value and acknowledge that clinical experience is equally important as well. We aim to improve our treatment methods by keeping updated with the latest evidence in our respective fields. Our team members regularly attend conferences, read research articles, present to members of the community and write educational blogs. We believe that being actively involved in learning is crucial and necessary as health providers.
